Description

Our support programs provide care and support for people living with HIV, AIDS and Hepatitis.

Services are available in Amador, Calaveras and Tuolumne Counties.

Mission:
We instill hope, foster knowledge, encourage sensitivity, and provide support in our communities for HIV & AIDS, other chronic diseases and related social issues.

Sierra HOPE Programs


All services are free and confidential.


HIV & AIDS Support Services
  • AIDS Drug Assistance Program (ADAP)- Enrollment in this state program provides needed medications for clients not covered by insurers or other government programs.
  • Case Management - Social worker and nurse case managers work together with the client to develop a care plan based on his/her specific needs. Case Managers provide benefits counseling and advocacy to assist clients in obtaining benefits and coordinate with other care providers to assure quality and continuity of care.
  • Emergency Financial Assistance - is available for HIV related medical needs for HIV/AIDS clients.
  • HOPWA (Housing Opportunities for People with AIDS) - short-term emergency assistance for rent, mortgage or essential utilities, to prevent or alleviate homelessness.
  • OA HIPP- Can pay medical insurance premiums for people with HIV who might otherwise lose their insurance coverage.
  • Transportation - Provided to clients for medical appointments by staff and volunteers or through use of transit tokens. Gas vouchers are available for clients that are able to transport themselves to medical appointments.

Prevention Education Services
  • Professional Education- Sierra HOPE will customize HIV/AIDS and Hepatitis presentations for in-service training sessions to meet the needs of health and human service providers.
  • Public Presentations - Trained speakers can provide an overview of AIDS, accurate and up to date information on HIV and Hepatitis transmission, prevention, treatment and care of persons living with AIDS.

Other Services
  • Community Food Pantry- Open to anyone in need of food, every Wednesday afternoon from 1:00 to 4:00 PM.
* Supportive Housing Program - Housing and case managment services are provided to qualifying indivduals or families who are disabled and homeless. This program is currently limited to six apartments.
